This is a letter I have used and you are welcome to use it or change it to fit your own needs. I have a question about the exact ingredients contained in this product: _________________________ [Fill in exact name and brand of product.]. The reason I write is because I follow the Specific Carbohydrate Diet (SCD), which has successfully treated my _________________ [Insert name of your illness]. The SCD successfully treats people with digestive diseases such as Crohn's Disease, Ulcerative Colitis, Celiac, and more. It is also used to help people with Autism. The catch to the diet, however, is that a person has to be on it 100% at all times for the diet to successfully eliminate all symptoms. I have been 100% on the SCD to treat my ____________________ [name of illness] for ___________________ [amount of time] with no medication or symptoms [or insert your own success here], and it has similarly helped thousands of others. 

The number of prepared foods that can be eaten by those on the SCD is very limited, and because we can't eat even a trace of something not on the SCD, we are all very careful about what prepared products we buy. Items we avoid: all starches (includes all grains, corn and starch), long-chain sugars (fruit sugar and honey are okay), and lactose. We are aware that companies aren't legally required to list on the label anything that makes up less than 2% of the product. But for those on the SCD, it's the 2% of food that could bring on many serious symptoms and possibly even require us to be hospitalized. And that is why I'm writing today to ask if you could please verify in writing whether there is nothing else in the ____________________ [Name of product again], other than the ingredients listed: ________________ [List ingredients].

Other concerns you might ask about are whether the product is produced on machinery that produces illegal items or whether the product is produced internationally then just packaged domestically. In certain instances, if it's produced in another country it may be hard to verify the production circumstances.
